Internship Forms

The information in FORM-1 will be filled in by the student and the company / institution and the signed version will be applied to the department commission and approval will be obtained for internship eligibility.

FORM-3 will be filled out by the internship supervisor (the person whose signature is in the internship notebook) in the relevant company / institution and sent to our department with a “confidential” record.

NOTE: The required certificate for the internship process will be obtained through the e-Government Gateway.
